Concierge APRN offers a 30-minute telehealth consultation for eligible Florida patients who want convenient, personalized medical care from home. Connect by video with a nurse practitioner for a focused virtual evaluation of new symptoms, ongoing health concerns, chronic-condition follow-up, medication questions or treatment planning. Your telemedicine visit may include review of symptoms, medical history, medications, allergies, supplements, previous diagnoses, laboratory results, treatment and wellness goals. Depending on the assessment, your provider may discuss possible diagnoses, treatment options, self-care, monitoring, preventive guidance, medication management, prescription refills, new prescriptions, laboratory testing, imaging, specialist referral or an in-person examination. A diagnosis, medication, prescription or refill is never guaranteed. Prescribing decisions are made only after an appropriate clinical evaluation and when medically indicated, legally permitted and within the nurse practitioner’s scope of practice. If medication is appropriate, an optional prescription add-on is currently available for $55. Payment for a consultation or add-on does not guarantee medication. A 30-minute virtual medical visit may suit patients seeking follow-up care, medication review, lab-result discussion, treatment-plan updates, non-emergency symptom evaluation, chronic-disease monitoring, preventive counseling or general medical guidance. Telehealth does not replace every physical examination, diagnostic test or procedure. If symptoms require hands-on assessment, blood work, imaging, EKG/ECG, urgent evaluation, emergency care or another level of treatment, an in-person visit or referral may be recommended. Concierge APRN telehealth services follow applicable Florida telehealth requirements, professional standards, evidence-informed protocols, privacy practices and scope-of-practice rules. Virtual visits are for non-emergency healthcare. Appointments may be available to eligible patients in Surfside, Bal Harbour, Bay Harbor Islands, Indian Creek Village, Miami Beach, North Beach, Mid-Beach, South Beach, Sunny Isles Beach, Golden Beach, Aventura, North Miami, North Miami Beach, Miami Shores, Biscayne Park, North Bay Village, Edgewater, Midtown Miami, Wynwood, Design District, Downtown Miami, Brickell and Hallandale Beach. Services are provided through professional mobile visits directly at your location as well as via telehealth consultations. Our consultation fee reflects the time, professional assessment, and thorough chart review provided. It is nonrefundable except when appointments are canceled more than 24 hours in advance, in which case a full refund or rescheduling is available. Cancellations made with less than 24 hours’ notice may incur a charge of 50% of the visit fee, while no-shows will be charged the full consultation fee. By booking an appointment, you authorize Concierge APRN, LLC to charge your payment method for applicable fees as outlined above. Patients are required to pay upfront for prescriptions; payment does not guarantee that a prescription will be issued, as prescriptions are provided only when clinically appropriate and within Florida scope of practice laws. If a prescription is paid for but ultimately not provided or not picked up, that portion may be refunded or credited toward a future telehealth visit or a suitable alternative. By scheduling a telehealth visit, you acknowledge and consent to receive healthcare services via electronic communication and understand that telehealth has limitations compared to in-person care and may not be appropriate for all conditions. Concierge APRN, LLC operates under Florida Statute 464.0123, granting autonomous practice authority to qualifying APRNs. This allows Dr. Kayla Azari Garmizo, DNP, APRN, to evaluate, diagnose, and treat patients without physician supervision within her scope of practice. All patient information is protected under the Health Insurance Portability and Accountability Act (HIPAA) and Florida privacy laws, and telehealth visits are conducted via HIPAA-compliant platforms to safeguard your confidentiality. By receiving services from Concierge APRN, LLC, you acknowledge that the information you provide regarding your medical history, medications, allergies, and current conditions is accurate and complete to the best of your knowledge, and you understand that Dr. Kayla Azari Garmizo, DNP, APRN, will base her treatment decisions on the information you provide. You release her from liability for any adverse outcome resulting from omitted, false, or inaccurate information you supply. Concierge APRN, LLC does not provide emergency medical services; if you are experiencing a medical emergency, call 911 or go to the nearest emergency department immediately. Supplements and vitamin services provided have not been evaluated by the Food and Drug Administration (FDA) and are not intended to diagnose, treat, cure, or prevent any disease.
